Police Records Request Form
Notice: There is a non-refundable $12 processing fee, due when submitting your request, which covers the first three (3) pages of any releasable record. You must pay this fee before your record can be processed.
An additional $1.50 will be charged for each additional page of a releasable record(s). Other fees that can be associated with records requests: Color Pictures: $1 each; Audio/Video CD copies: $25 each. We will contact you by phone and advise you when you can pick up your record and if there are additional fees.
Please note that it could take 1-2 weeks or longer to process your request. If you need an immediate response to your request you may call dispatch at 541-938-5511.
Please note that all juvenile information is redacted per ORS requirements.
