These forms are provided for your convenience in Adobe Acrobat PDF. Adobe Reader is required to view the forms and may be downloaded from Adobe.com if you do not already have it. Forms are also available through City Hall.

1. If you have questions on which form to use, please call City Hall  (541-938-5531)
2. Click on the form name to view the necessary form.
3. Print the form(s).
4. Fill out the form completely. Incomplete or incorrect forms will not be accepted.
5. Forms designated by * require the customer to submit them in person. Otherwise, forms may be mailed to City Hall.
